Podiatrist or Foot Care Nurse? Who Should I Choose?

Do I need a podiatrist, or can I see a foot care nurse?
It’s a great question—and the answer really depends on what is happening with your feet.

Here’s a simple way to understand the difference.
What Does a Foot Care Nurse Do?
A foot care nurse is a Registered Nurse (RN) or Registered Practical Nurse (RPN) who has additional education and experience in foot assessment and nursing foot care.
Foot care nursing is very different from a cosmetic pedicure.
During an appointment, your nurse looks at the whole foot, not just the toenails.

Assessment of the skin, nails and overall condition of the feet
Trimming and filing difficult or thickened toenails
Reduction of thick or fungal-appearing nails
Corn and callus care
Care of dry or cracked heels
Conservative care for some ingrown toenails

Monofilament testing when appropriate
Education about footwear, pressure, skin care and nail care
Identification of changes that may need assessment by another healthcare professional
Ongoing preventative foot care
For many people—especially older adults, people living with diabetes, or people who simply cannot safely reach or care for their own feet—an RN foot care nurse is an excellent place to start.
What Does a Podiatrist or Chiropodist Do?
In Ontario, chiropodists and podiatrists are regulated foot-health professionals who assess, diagnose and treat medical conditions affecting the feet. Their scope may include treatments and procedures that are outside the scope of routine nursing foot care.
They may be particularly appropriate when a client requires investigation or treatment of a more complex foot disorder, biomechanical problem or condition requiring services outside a foot care nurse's scope.
Depending on the individual practitioner and clinical situation, this may include concerns involving:
Persistent or unexplained foot pain
Structural or biomechanical problems
Significant foot deformity
Orthotics
Certain medications or prescriptions
More complex wounds
Procedures requiring an expanded scope
Conditions requiring a formal diagnosis
Your family physician may also be an important part of the picture, particularly when a foot problem could be related to infection, circulation, diabetes, medication or another medical condition.

Here are some common examples.
“My toenails are thick and I can’t cut them anymore.”
A foot care nurse may be a great choice.
We routinely care for thick, difficult and fungal-appearing nails and can safely reduce and trim them while assessing the surrounding skin and foot.
“My parent can no longer reach their feet.”
A foot care nurse is often an excellent option.
This is one of the most common reasons families contact us. We provide both clinic appointments and foot care home visits throughout Ottawa, including retirement residences, hospitals and long-term care settings.
“I have diabetes and want someone to regularly check my feet.”
A foot care nurse can play an important preventative role.
Our nurses assess the feet, monitor for changes, provide appropriate nail and skin care, offer education and identify concerns that should be referred to the client's physician or another foot-health professional.
“I have a painful corn or a lot of hard skin.”
A foot care nurse may be able to help, depending on the assessment.
We can provide conservative corn and callus care and talk about possible pressure or footwear contributors. If something doesn't look typical or requires more specialized assessment, we'll recommend appropriate follow-up.
“I have ongoing foot pain when walking.”
This may require assessment by a physician, chiropodist, podiatrist, physiotherapist or other appropriate healthcare professional, depending on the symptoms.
Foot care nurses can identify and document concerns, but persistent or unexplained pain often needs further investigation.
“My toe is suddenly red, hot, swollen or draining.”
Signs of possible infection or an acute change should be medically assessed. Depending on the situation, we may recommend contacting your physician, nurse practitioner or another appropriate healthcare provider before or alongside foot care.
